Comments:
An unannounced monitoring inspection was conducted on Septemeber 20, 2022 from approximately 10:35 am-11:37am. There were 10 children present at this time with 3 staff members. Children were observed participating in circle time , singing and working on writing and coloring. Three staff records, three children's records, no injury report and no medications were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication
procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on observation ans staff interview, the licensee did not ensure the carbon monoxide detector was in the buildingins that serves preschool - age children.

Evidence:
The owner confirmed the center does not have a carbon monoxide detector.

Description: Based on record review, the licensee did ensure at least every two years from the date of the first initial screening or testing staff members shall obtain and submit results of tuberculosis (tb) screening.

Description: Based on observation and staff interview, the licnesee did not ensure a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.
