63.2 Child Abuse & Neglect

Comments:
An unannounced renewal inspection was conducted today beginning at 2:00pm. At the time of entrance, 25 children were in care with 3 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, the Emergency Preparedness Plan, 2 children's records and Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The site was clean and organized. The children were observed checking in and free play. Standard #: 8VAC20-780-60-A
Description: Based on review of children's records, the center's files did not contain all information required.
Evidence:
1. Child's 2's (start date 8/25/2022) file was missing the parent's place of employment and previous schools attended.

Plan of Correction: I will ask my supervisor to contact them for the information.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of the allergy list, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #2 (start date 8/25/2022) had a diagnosed food allergy, but did not have a written care plan fo include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: I will ask my supervisor to contact them for the information.

Standard #: 8VAC20-780-550-P
Description: Based on review of records, the center had a minor injury record that did not include all required signatures.
Evidence:
1. Child #3 had an accident report dated 9/09/2022 that was signed by one staff member only.

Plan of Correction: I will make sure in the future to either have the parent sign or another staff sign with me.
