Comments:
An unannounced monitoring inspection was conducted on 9/29/2022 from 2:50 pm-3:30 pm. There were 31 children supervised by 2 direct care staff. The physical plant, 1 staff record, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-780-510-E
Description: Based on medication review, long-term prescription drug use and over-the-counter medication were allowed without written authorization from the child's physician and parent.

Evidence: Child #6 had a written authorization form for long-term medication signed by their parent, but the physician's signature, as well as the medication name, dosage, route, and effective dates, were missing from Child #6's written authorization form.

Plan of Correction: I will speak with parent today at pick up. Parent was given long form to complete should have back ASAP or by 10/10/2022
