Technical Assistance:
When parents fill out medication authorization forms, you will need to pay close attention to the effective duration dates to make sure they are accurate and if not, please have the parent make the necessary changes/corrections.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with two additional staff and 13 children. The children and staff were outside on the playground at the beginning of the inspection. For their snack, the children were provided cheese crackers and milk. Medications were reviewed for five children, two allergy plans were also reviewed.
Three staff files and five children's files were reviewed today. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of children's files and interview with the director, it was determined that child A did not have an immunization record on file before the child attended the program.
Child A enrolled on 8/22/22.

Plan of Correction: I will obtain the immunization record.

Standard #: 8VAC20-780-140-A
Description: Based on a review of children's files and interview with the director, it was determined that child A and B did not have physicals on file within 30 days of enrollment. Both children enrolled on 8/22/22.

Plan of Correction: I will obtain the physicals.
