Technical Assistance:
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated September 28, 2022. Once the background check is received by the center, the results must be forwarded to the licensing inspector.

All renewal applicant's background checks must be current for a renewal license to be issued. Applicant 2's central registry check is due to expire soon. It is recommended that a new one is requested as soon as possible to avoid delays in license issuance.

Comments:
An unannounced renewal inspection was conducted today from 10:30am-12:30pm. There were 42 children directly supervised by10 staff. The physical plant, 7 staff records, 6 children?s records, 4 renewal applicant records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, being fed, playing outside, and transitioning between activities. Diapering and hand-washing procedures were also observed. Staff and child interactions were observed to be constructive in nature and age appropriate. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289-036-B-4
Description: Based on record review, renewal applicants did not have the results of an out of state child abuse and neglect search by the end of their 30th day of employment, or the results of a criminal record check prior to the first day of service.
Evidence:
1. Applicant 1's record does not contain the results of an out-of-state criminal record check for two listed out of state residences.
2. Applicant 2's record does not contain the results of an out-of-state criminal record check and
an out- of-state central registry check for two listed out of state residences.

Plan of Correction: Applicant 1 and 2 have requested these out of state records-we have 45 days to
obtain these records according to VA state regulations.

Standard #: 8VAC20-770-40-D-1-a
Description: Based on record review, one renewal applicant did not have the results of a central registry check on file.
Evidence:
1. The central registry check for Applicant 1 expired on 7/8/2021.
2. The sworn statement for Applicant 3 expired on 9/8/2022.

Plan of Correction: 1.) A new central registry form has been submitted on Applicant 1.
2). Applicant 3 Sworn Statement has been updated and forwarded to the licensing agent.

Standard #: 8VAC20-780-50-A
Description: Based on observation children's records were not treated confidentially.
Evidence:
A list containing confidential student medical information was observed posted in publicly observable areas in the Older Toddler and Older Infant classrooms.

Plan of Correction: All teachers have been instructed on the importance of confidentiality. The
teachers have been instructed to put all information regarding medication/health related items on the
inside of the locked cabinet door. This has been classified as a ?null risk assessment.? We are unique
that we are on a federal location and our classrooms are restricted to public access. In fact, parents are
only allowed in the hallways and not in the classrooms.

Description: Based on observation, areas of the center were not maintained in a safe condition.
Evidence:
A structural wooden platform in the Older Toddler classroom was observed with a chip in the wood approximately the size of a dime. The edges of the hole were sharp to the touch.

Plan of Correction: This is not a repeat violation of the last 2 visits on 6/9/22 and 12/21/21.
However, because it can pose a hazard to the children, a work order was submitted to the Federal
Government. They came in on Friday, Sept. 30 th and fixed this hole.

Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A canister of disinfectant wipes was observed on a low counter in the Preschool classroom.
2. A bottle of spray cleaner was observed in an unlocked cabinet in the Young Toddler classroom.

Plan of Correction: Hazardous Substances-1. The teacher had just purchased this for the classroom
and left it on her desk area. She was reminded of making sure all cleaning materials need to be in a
locked cabinet immediately.
1) The canister was headed to the recycle unit located in the kitchen area. The teacher was
planning on taking this out during her break. The teacher was reminded to keep these types of
materials higher up and out of the reach of children, until it can be disposed of properly.
2) The unlocked cabinet has been submitted to the Federal government to replace the lock, as it is
broken.

Evidence:
An unlabeled bottle containing a green liquid that a staff member identified as "cleaning solution" was observed in the Young Toddler classroom.

Plan of Correction: All classrooms have been given new labels to clearly indicate what cleaning
solution is in a particular bottle. They also have date labels added to the bottles to address any other
possible violations.

Standard #: 8VAC20-780-430-B
Description: Based on observation, materials available were not age and stage appropriate for the children in care.
Evidence:
1. In the Older Toddler Classroom an open shelving unit was observed to contain open and accessible jars and containers of: pony beads, small rocks, small balls, glitter, and metallic confetti, which pose a choking hazard for children under the age of three.
2. In the Older Infant room an open container of small "googly eyes" was observed on a low shelf accessible to children, and posed a choking hazard.
3. In the Infant classroom, an unsealed sensory bottle containing dry rice and beans, and a canister containing small plastic pieces, were observed on open shelves accessible to children.

Plan of Correction: After 21 years (and never being cited for this particular item) of having a Reggio
Sensory shelf in the Older Toddler room, the materials on the shelf and the shelving unit have been
removed. Violation #2 and #3 were addressed with staff and these items have been removed from these
rooms. Sensory bottles are to be sealed properly, with glue.

Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Liquid allergy medication and an epinephrine injector were observed inside of an unlocked storage box in the Older Toddler Classroom.

Plan of Correction: Medication storage: A new locked box was given to the teachers. This
medication box is and was located in a locked cabinet on the second shelf of an upper cabinet.

Description: Based on observation, sunscreen was not kept in an area inaccessible to children.
Evidence:
Sunscreen was observed stored in a zip lock bag in a child's cubby in the Preschool classroom.

Plan of Correction: An email was sent on Wednesday, Oct. 5 to the parents and teachers
reminding them that all over-the-counter products, such as sun screen, bug repellent, diaper cream
and/or lotions must be handed directly to the teacher and an over-the-counter form must be filled out
and accompany the product when the product is handed to the teacher.
