Comments:
An unannounced monitoring inspection was conducted on Wednesday, September 27, 2022, to determine the center's compliance with licensing standards. The inspection begin at 3:10 PM and concluded at 4:50 PM. The center's director was available throughout the inspection. The census for today's inspection consisted of 115 children in care and 7 staff supervising. During the tour, the inspector observed the children eating snack, playing outside, meeting in the gym, and interacting with peers and staff. The inspector observed all inside and outside play areas, bus, and restrooms. The center's first aid kit and non-medical emergency supplies were inspected and found complete. The required postings were inspected and found to be in compliance. Liability insurance, health inspection, and drills were all in compliance. A total of 10 child records and 8 staff records were reviewed. Staff records were reviewed remotely. Violations were cited and can be found in the violation notice.

Standard #: 8VAC20-780-130-A
Description: Based on a review of records, 4 of 10 children's records were missing documentation of immunizations prior to the child's first date of attendance. Evidence: 1. The record for Child #1 [date of enrollment 8/29/22] child #2 (date of enrollment 8/29/22), child #6 (date of enrollment), and child # 9 (date of enrollment), did not include documentation of immunizations.

Description: Based upon a records reviewed, one staff record did not contain documentation of a TB screening or test within 30 days of employment. Staff #1?s record did not contain an official Tuberculosis test (DOH 6/27/2022).

Description: Based on staff and children's interviews, children under the age of 10 years were not within actual sight and sound supervision of staff. Evidence: During the inspection 2 children age 6 & 7 were observed walking alone in the hallway from the cafeteria to the restroom. Children were not within sight and sound of staff. Staff confirmed the child came from the cafeteria and should not have been in the hall unsupervised.
