A monitoring inspection was conducted on 9/29/2022 with the center director and one staff. There were 14 children present, ranging in ages from 5 years to 12 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 0 child records and 0 staff records were reviewed. The children were finishing homework and preparing to go outside.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on staff interview, the facility failed to have and keep at the center a record for each enrolled child. Evidence: the director did not have his binder or his laptop with the children's files on site today. He stated the school has information on each child, but the office closes at 4:30 p.m. and the center closes at 6:00 p.m. Plan of Correction: Children's records will be brought to the center daily.
Standard #: 8VAC20-780-530-A-1 Description: Based on staff interview, the facility failed to have one staff in each in each grouping of children with cardiopulmonary resuscitation (CPR). Evidence: Staff 1 was alone in Room 29 with 13 school age children, but does not have current CPR. Plan of Correction: Children will not be left alone with staff who are not current in first aid and CPR.
Standard #: 8VAC20-780-530-A-2 Description: Based on staff interview, the facility failed to have one staff in each in each grouping of children with first aid certification. Evidence: Staff 1 was alone in Room 29 with 13 school age children, but does not have current first aid certification. Plan of Correction: Children will not be left alone with staff who are not current in first aid and CPR.
