**DIscussed with the site director that they need to ensure that the reference checks completed on new staff members were not done with relatives. One staff member had a reference check completed by a relative.

An unannounced renewal inspection was conducted on 9/26/22 from 4:05pm to 5pm. There were 15 school-agers directly supervised by 3 staff. The physical plant, 2 staff records, 5 children?s records, 1 child's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group and individual socialization and play, and preparing to play outside. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report.
