Technical Assistance:
1. Form provided: Emergency preparedness plan, proof of identity.

Comments:
10:45am-12:00pm Annual unannounced subsidy inspection conducted. Observed 2 children = 4 points with the vendor. Ratios and supervision were in compliance. The children were playing with toys and watching child appropriate cartoons on TV during the inspection. Transportation provided daily. The home was found to be very clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The vendor is currently certified in First Aid/CPR. Recordkeeping non-compliances found.

Description: Based on records reviewed, 5 of 5 children's records reviewed did not include required information.

Plan of Correction: The vendor will have parents complete the required paperwork.
