Technical Assistance:
Technical Assistance was given and standards were reviewed regarding physical plant.

Comments:
An unannounced monitoring inspection was conducted on October 6, 2022 from 12:40pm to approximately 2:25pm. There were 15 children present and three staff members. A sample size of 9 records were reviewed which included six children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during bottle feeding and naptime. First aid kits and emergency preparedness documents were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview, it was determined that the center did not ensure that there is a physical examinations for children. Each child shall have a physical examination by or under the direction of a physician: 1. Before the child's attendance; or 2. Within 30 days after the first day of attendance.
Evidence:
1. Child #4 (date of enrollment 2/20/2022) did not have a physical examination in their record.
2. Staff #1 confirmed that Child #4 did not have a physical examination to their record.

Plan of Correction: The center responded with the following: Staff #1 will obtain the physical examination for Child #4 and place it in their record at the center.

Standard #: 8VAC20-780-60-A
Description: Based on record review and interview, it was determined that the center did not ensure that each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. Child #3 (date of enrollment 9/08/2022) did not have an address or phone numbers listed to their emergency contact.
2. Staff #1 confirmed there was no emergency contact listed for Child #3.

Plan of Correction: The center responded with the following: Staff #1 will obtain the address and the phone number for Child #3 at pick up and will document the information in the child?s record.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The hallway sink countertop has a container of disinfectant wipes with a warning label, ?Harmful to humans? that was in reach of children.
2. Near the changing table in the toddler room was a container of disinfectant wipes in reach of children.
3. Staff #1 and Staff #2 confirmed that the containers of disinfectant wipes were not locked and in reach of children.

Plan of Correction: The center responded with the following: Staff #1 immediately moved the container and placed it in a locked cabinet inaccessible to children.

Standard #: 8VAC20-780-440-G
Description: Based on observation and interview, it was determined that the center did not ensure that cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat.
Evidence:
1. Child #4, age 25 month, was sleeping in a crib during naptime.
2. Child #4 did not have a physical examination report or documentation in their record to support not being developmentally ready to sleep on a cot or mat.
3. Staff #1 confirmed that there is no documentation by a physician or parent stating that the child can remain in a crib.

Plan of Correction: The center responded with the following: Staff #1 will speak with Child #4?s parent regarding the licensing standard requirements and have the appropriate documentation to support the child sleep in a crib or have the child transition to a cot or mat.

Standard #: 8VAC20-780-520-C
Description: Based on observation and interview, it was determined that the center did not ensure that if diaper ointment or cream is used, the following requirements shall be met: these products shall be in the original container and labeled with the child's name.
Evidence:
1. The cabinet near the changing table had two containers of petroleum jelly that were not labeled with a child?s name.
2. Staff #1 confirmed that there was no labels to the containers with the child?s name.

Plan of Correction: The center responded with the following: Staff #1 will ensure that all creams/ointments are in the original container and labeled with the child?s name.

Standard #: 8VAC20-780-570-B
Description: Based on observation and interview, it was determined that the center did not ensure that bottles shall not be used while the child is in his designated sleeping location.
Evidence:
1. Child #1 was observed sleeping in their crib and their bottle was beside them inside the crib.
2. Child #2 was observed crying in the crib and was given a bottle during nap time while inside the crib.
3. Staff #3 confirmed that the bottles were in the crib for both Child #1 and Child #2.

Plan of Correction: The center responded with the following: Staff will no longer give the children their bottles in their designated sleeping locations. Staff immediately removed the bottles out of the children?s cribs during the time of the inspection.
