A complaint was received in the licensing office on 9/8/2022 regarding consistency of care. On 9/15/2022 an on-site investigation was conducted. Interviews were conducted, records were reviewed, and observations were made. At the time of this investigation, the preponderance of evidence did not support the allegation and the complaint is deemed not valid.
