Technical Assistance:
Discussed new orientation requirements and recomended the center to use the model form for the staff records.

Comments:
An unannounced monitoring inspection was conducted on October 6, 2022 approximately 6:34 am-9:42 am. There were 11 children present at this time with 5 staff members. Children were observed participating in table activities and moving freely in the classroom , children arriving and children depature on to the bus. Three staff records, three children's records, five injury report and two medications were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented onvthe violation notice issued to the program.

Description: Based on staff interview and observation, the licensee did not ensure there was a carbon monoxide detector in the building.

Evidence:
The director/owner confirmed she was not sure if they had a carbonmonoxide detector.

Description: Based on record review, the licensee did not enusre staff central regisrty findings / results were completed with in 30 days.

Evidence: The center did not have staff #3 central registry results during the inspection. Staff #3 hire date was 06/13/2022.

Description: Based on observation, the licensee did not ensure hazordous substances were kept in a locked place using a safe locking method that prevents access by children .

Evidence:
In the infant room and toddler room , there was alchol free handsanitizer that were labeled with warning and keep out of reach of children. The hand sanitizer was not kept in a locked place.
