Comments:
A monitoring inspection was completed on 10/05/22. There were 17 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and were reviewed. The inspection started at 4;30pm and concluded at 5:45pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Thank you for your cooperation.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the center failed to have a physical examination by or under the direction of a physician for children in care.
Evidence:
1. Five children's records were reviewed. All five records had a student health information form completed by parents, not a a physical examination by or under the direction of a physician.

Plan of Correction: The director will contact school staff or the parents to get the physicals.
