Comments:
An unannounced monitoring inspection was conducted on 10/4/2022 from 4:20pm to 5pm. There were 18 school-agers directly supervised by 3 staff. The physical plant, 4 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group and individual play. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Description: Based on review, the facility did not ensure that they obtained documentation that each child has received the immunizations required by the State Board of Health.
Evidence: reviewed 5 children's records and found that Child 1 and Child 4 did not have documentation of an immunization record.

Description: Based on observation, the facility did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed 2 bottles of food contact sanitizer accessible to children in the main activity space on a trolly near the back wall.
