The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 10/19/2022 relating to supervision during an incident on 10/05/2022 and accident report not being signed and given to the parent in the school age classroom on 7/15/2022. Interviews were conducted with staff, video footage was viewed from 10/05/2022, the 7/15/2022 accident report was reviewed and one staff record was reviewed.
The information gathered during this investigation regarding supervision does not support the allegation.
However this investigation does support the allegations of an accident report not including all required information and signatures on the injury report, so the complaint is determined to be valid. See the Violation Notice for findings.

Standard #: 8VAC20-780-550-P Complaint related: No Description: Based on review of a written record of child's serious and minor injury report, the center did not include all required information in the report record.
1. Child #1 had an injury report that did not include the time of injury, date and time parents were notified, staff and parent signature or two staff signatures and how the parent was notified.
Plan of Correction: Director has spoken with staff and explained the importance of correctly filling out incident reports. Retraining was done on 10/25 & 10/26 to ensure reports are filled out properly.
