Comments:
An announced non-mandated inspection was conducted in response to a self-reported incident that occurred on 10/07/2022. Areas of non-compliance are identified on the Violation Notice. For questions regarding this inspection, please contact stephanie.salvatori@doe.virginia.gov

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on information obtained during an inspection conducted on 10/13/2022, it was determined the licensee did not ensure they followed their own policy regarding child supervision. Evidence: 1) Under the facilities? Safety, Accountability, Supervision Procedures, it states staff must ?Supervise all children at all times. In sight and sound.? 2) The facilities? policy was not followed by Staff A or Staff B, during an incident which occurred on 10/07/2022.

Plan of Correction: All staff will follow SACC program policies and procedures for the supervision of children. Groveton Center staff will receive additional training on SACC?s Safety, Accountability and Supervision procedures, including active supervision and the consistent and focused implementation of procedures for sight and sound supervision and accountability during transitions. The center specific safety and accountability plan will be reviewed and updated. The Regional Supervisor will review the updated center safety and accountability plan and conduct periodic site visits to ensure the consistent implementation of program policies and procedures for supervision and accountability.
All SACC center staff will participate in additional training on SACC?s Safety, Accountability and Supervision procedures to ensure attention on focused implementation of the procedures program wide and the safety of all children.

Standard #: 8VAC20-780-340-A
Description: Based on information obtained during an inspection on 10/13/2022, it was determined the licensee did not ensure the care, protection and guidance of a child in the facility. Evidence: 1) On 10/07/2022, Child #A was left alone in the bathroom while the remainder of the class proceeded outside towards the playground. 2) The exit of the bathroom is not visible from the door leading outside. 3) The distance from the bathroom exit to the outside door is approximately 28 feet. 4) While standing approximately 34 feet on the other side of the glass door, Staff A saw Child A turning the corner from the exit of the bathroom, approaching Staff A, to come towards the door leading outside. 5) Child A (age 5) was unharmed, not upset and alone for less than 1 minute.

Standard #: 8VAC20-780-340-F
Description: Based on information obtained during the inspection, it was determined the licensee did not ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff, except that the staff need only be able to hear a child who is using the restroom. Evidence: 1) Child A was not within sound of staff while using the bathroom. 2) Staff A proceeded outside while Child A was still in the bathroom. 3) A glass door, and approximately 62 feet, separated Staff A (outside the glass door) from Child A (inside the bathroom).
