Comments:
An unannounced renewal inspection was conducted on 10/14/2022 from 9:00am to 11:30am. There were 13 children ages 18 months to age 4 years old supervised by 6 staff. The physical plant, outdoor playground area, programming, menus, 3 staff records, 3 children?s records, emergency drills, 2 injury reports, and emergency supplies were reviewed. There were no medications onsite. Children were observed participating in dramatic play, playing with cars, and eating. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in MAT, CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1)In the kitchen area off of the main hallway there is no door and the cabinet under the sink was unlocked and contained 2 gallons of bleach.
2)In the teacher resource area off of the main hallway there is no door and there was a box of bleach sitting in the entryway to this area.
3)In the janitorial closet off the main hallway, the door was unlocked and there was a multi-surface cleaner, bleach and glass cleaner located inside the closet.

Plan of Correction: All kitchen cabinet locks under the sink were replaced and locked securely. All hazardous substances were relocated from the hallway to the janitorial closet. The janitorial closet was locked with a key inaccessible to children. Also, an email reminder was sent out to our operations teams that monitors our cleaning crew to make sure that the janitorial closet is closely monitored to ensure that it is locked about being utilized.

Description: Based on observation and staff interview, the center did not ensure that food brought from home was clearly dated and labeled in a way that identifies the owner.

Evidence: In the Infant classroom, Child #4 had breast milk that was stored in the refrigerator and was not dated or labeled with the child?s name.

Plan of Correction: Center manager reminded all staff with children in the Infant Classroom to accurately date and label their infant's breast milks before being refrigerated. Infant teachers should also review and confirm labeling. Breast mill bottle was immediately labeled by infant teacher while licensing inspector was on site.
