Technical Assistance:
Standard 420.E.3 regarding semi-annual written updates to parents regarding their child's development was discussed with the program director.

Comments:
A monitoring inspection was conducted on 10/20/2022. There were 1 children present, ranging in ages from 2 years to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based upon review of children's records and staff interview, the facility has not ensured that each child's record includes the names, addresses and telephone numbers of two persons to be contacted in an emergency if a parent cannot be reached.
Evidence:
1. The record provided for child 1 did not include any persons to be contacted in an emergency other than the parents.
2. Staff 1 verified that the record for child 1 did not include emergency contact persons other than parents.

Plan of Correction: The facility responded with the following during the exit interview:
All children's records will be checked to ensure that the names, addresses and telephone numbers for at least two persons to be contacted in an emergency when a parent cannot be reached is included in each record.

Standard #: 8VAC20-780-530-A
Description: Based on staff interview, the facility has not ensured that at least one staff person with first aid and CPR certification is in each area where children are present.
Evidence:
According to staff 1, staff 2, who drives the van to pick up children from elementary school, does not yet have first aid/CPR certification.

Plan of Correction: The facility responded with the following during the exit interview:
The van driver has already been scheduled to obtain first aid and CPR certification.

Standard #: 8VAC20-780-540-A
Description: Based upon staff interview, the facility has not ensured that there is a first aid kit wherever children are in care.
Evidence:
Staff 4, a van driver, and staff 1 both stated that a first aid kit has not been taken on the van when picking up children from school.

Plan of Correction: The facility responded with the following during the exit interview:
A complete first aid kit will be prepared and placed in each vehicle used to transport children.
