Comments:
An unannounced on-site renewal inspection was conducted on 09/26/2022. There were 24 children, ages 3 years-5 years, and 4 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play, and lunch. A total of 5 children?s records and 4 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety.

Violations:
Standard #: 22.1-289-036-B-4
Description: Based on review of five staff?s records, the center failed to ensure that all background checks were completed as required.

Evidence:
1. Staff # 2 was hired 08/11/2022 and lived outside of the state of Virginia in the past five years. The center did not obtain the CPS from the state that staff # 2 lived. The state did participate in a NFF (National Fingerprint File).

Plan of Correction: The administration will have the out of state CPS completed ASAP.

Standard #: 8VAC20-780-40-J
Description: Based on review of the injury report the center failed to ensure that the plan was reviewed annually.

Evidence:
1. The most recent review was conducted 8/2021.

Plan of Correction: Head of School will update the injury plan annually and will ensure that all staff are trained.

Standard #: 8VAC20-780-40-K
Description: Based on interviews with the director, the center failed to ensure that they developed a written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Plan of Correction: The Head Of School will develop the policy and will make sure that all staff are aware.

Description: Based on review of five children?s records, the center failed to ensure that all records were complete.

Evidence:
1. Child # 1 did not have a complete address for one of the two required emergency contacts.

Plan of Correction: The staff will have the parent to add the address and will monitor all records.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all chemicals were locked.

Evidence:
1. In the Preschool room, the chemicals were stored under the teacher?s desk unlocked.

Plan of Correction: The chemicals were locked during the inspection.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center failed to ensure that all medications were locked.

Evidence:
1. There were four containers of children?s medication that was not locked.

Plan of Correction: The medications were taken back to the main school building and will not be located at the Nest.

Standard #: 8VAC20-780-550-P
Description: Based on review of the injury reports, the center failed to ensure that all required items were obtained.

Evidence:
1. The time the injury occurred and how and method the parent was notified was not obtained on each injury report.

Plan of Correction: The center will use the model form on the DOE website.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the center failed to ensure that the tables were cleaned and sanitized as required.

Evidence:
1. Observed the preschool tables not being cleaned at least once a day. The staff was using only a sanitizing wipe and not a cleaning agent.

Plan of Correction: The center will start using soap and water in a spray bottle to meet the standard.
