An unannounced monitoring inspection was initiated and concluded on 10/24/2022. There were 14 children present with 3 staff supervising. The physical plant, children's records, staff records, evacuation drills, first aid kits, menu's, injury reports, medications and medication authorizations were reviewed. Areas of non-compliance were cited on the violation notice. The children were observed playing with floor toys, making bean bags and playing games with the staff. Child #3 and child #4's records do not contain documentation of a physical examination by the 30th day of attendance, Child #3 and #4's first day of attendance 8/23/21.Plan of Correction: I will contact parents and request a copy of the physical.
