A mandated monitoring inspection was initiated was conducted on October 25, 2022 from 9:40 A.M.-1:30 P.M. There were 81 children present, ranging in ages from six months to five years of age, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and ten staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on review of children records, the center failed to ensure that all information was in the children's records.
Evidence:
1. The records for Child #2, Child #3 and Child #6 did not contain information regarding who has legal custody of the child.
2. Administration verified that the information was left blank and not completed on the registration form.Plan of Correction: Administration will contact parents and request the information for the children's files. In the future all registrations will be audited to ensure all information is in the record.
Standard #: 8VAC20-780-530-C Description: Based on observation, interview, and review of staff records, the center failed to ensure that at least two staff who are certified with CPR and First Aid were on a field trip with a group of children.
Evidence:
1. Classroom #1 was observed on a walking field trip. Staff #1 is not currently certified in CPR and First Aid.
2. Staff verified that they were not currently certified in CPR and First Aid.Plan of Correction: Staff will be reminded that when out on walks they need to have two staff that are currently certified in CPR and First Aid. A training will be scheduled for the staff member to obtain the certification.
Standard #: 8VAC20-780-540-A Description: Based on observation and interview, the center failed to ensure that a first aid kit was taken on
walking field trips.
Evidence:
1. Classroom #1 was observed leaving the center for a walk. The did not have their emergency backpack which contains the first aid kit with them.
2. Staff #1 and Staff #2 verified that they did not have a first aid kit with them on the walk.Plan of Correction: Staff will be remined than when they leave the classroom for a walk or field trip that the emergency backpack must go with them, as it contains the First Aid Kit and emergency contact information.
