Comments:
An investigation was initiated on August 1, 2022, with local agencies in response to a report received from the facility by the Fairfax Licensing Office on July 29, 2022. Interviews with staff, observation of program, and auditory observation of taped interviews were jointly reviewed relating to allegations of abuse. The information gathered during this investigation supports the allegations and has determined non-compliance with applicable standards or law. Violations:
Standard #: 8VAC20-780-340-A
Description: Based on auditory observation of video-taped interviews, it was determined a staff member did not ensure a child?s care, protection, and guidance while supervising children.
Evidence:
1. Staff #1 stated that:
a. Staff #1 touched Child #1 (6 years old) under Child #1?s clothes and Child #1?s private area on one occasion while supervising children in a classroom; and
b. It only happened on one day, Thursday July 28, 2022.
2. Child #1 stated that:
a. Staff #1 would have Child #1 come sit on Staff #1?s lap while other kids were playing in the same classroom, and touched Child #1 under Child #1?s clothes; and
b. Staff #1 has touched Child #1 under their clothes more than once.

Plan of Correction: Immediately after learning of the incident, The Campagna Center removed and subsequently terminated the employee. The Campagna Center self-reported the incident to our licensing specialist, Child Protective Services and fully cooperated with the Alexandria Police Department. Staff received training on child abuse and neglect before working with children, and on an annual basis. All program staff received additional training in Function of the Center-Roles and Responsibilities and Function of the Center- Forbidden Actions when engaging with Children on October 10th. Staff will receive an additional training on recognizing and reporting suspected child abuse on October 25th.

Standard #: 8VAC20-780-400-A
Description: Based on auditory observation of video-taped interviews, it was determined a staff member did not interact with a child that provided help, comfort, and respect personal privacy.
Evidence:
1. Staff #1 stated on one occasion in the classroom, on Thursday July 28, 2022, Staff #1 used one hand to hold Child #1 (6 years old) and placed the other hand under Child #1?s clothes and touched Child #1?s private area.
2. Child #1 stated that:
a. Staff #1 had Child #1 bring Child #1?s tablet over to the desk where the staff member was sitting and had Child #1 sit on Staff #1?s lap.
b. Staff #1 held Child #1 and when Child #1 tried to get up, Staff #1 pulled Child #1 back and put one hand under Child #1?s clothes.
c. Staff #1 washed their hands after Staff #1 let Child #1 get up.
d. This happened more than once and Child #1 did not like it.

Standard #: 8VAC20-780-410-1
Description: Based on auditory observation of video-taped interviews, it was determined a staff member
roughly handled and forced a child to assume an uncomfortable position.
Evidence:
1. Staff #1 stated that on one occasion, on Thursday July 28, 2022, Staff #1 placed one hand under the clothes of Child #1 (6 years old) and touched Child #1?s private area.
2. Child #1 stated that:
a. Staff #1 sat in a chair behind the desk and had Child #1 sit on Staff #1?s lap when Staff #1 touched Child #1 under their clothes;
b. When Child #1 tried to pull away from Staff #1 or stand up, Child #1 was pulled back to the seated positon on Staff #1?s lap;
c. This happened more than once; and
d. Child #1 ?didn?t like it?, ?it feels bad?, and ?it sometimes hurt?.

Plan of Correction: Forbidden Actions, as listed in the licensing standards are reviewed with staff at orientation and in trainings throughout the year. Immediately after learning of the incident, The Campagna Center removed and subsequently terminated the employee. Forbidden Actions were reviewed with All Campagna Kids Staff on October 10th and will be reviewed again on October 25th.
