An unannounced inspection was conducted on October 27, 2022 from 7:55 am to approximately 10:30am. There were 7 children present with 3 staff members. A sample size of 11 records were reviewed which included six children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed coming into the center during morning check in, during indoor and outdoor free play, diapering, and handwashing. First aid kits and emergency preparedness documents were observed. The transportation vehicle was observed. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the site director/administrator at the exit interview.

Standard #: 8VAC20-780-60-A Description: Based on record review and interview, it was determined that the center did not ensure that each center shall maintain and keep a separate record for each child enrolled which shall contain the work phone number of each parent who has custody.
Evidence:
1. Child #1 (date of enrollment 10/04/2021) did not have their parents? work phone numbers in their record.
2. Child #2 (date of enrollment 4/20/2022) did not have their mother?s work phone number in their record.
3. Staff #1 confirmed that Child #1 and Child #2 did not have the work number of their custodial parent.Plan of Correction: The center responded with the following: Staff #1 will obtain the work phone numbers for the parents.
