22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Comments:
An unannounced renewal study was conducted from 1:00 pm - 3:00 pm. There were 49 children in care being supervised by 11 staff members. The children were observed playing outdoors, engaging in story time, and napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-A
Description: Based on review of four staff records it was determined that not all background checks were repeated every five years. Evidence: The most recent central registry check on file for Staff B is dated 9/29/2017.

Plan of Correction: Administration will inquire with the Office of Background Investigation the status of the central registry check for Staff B.

Standard #: 22.1-289.035-B-2
Description: Based on review of four staff records it was determined that not all records contained documentation of a determination letter based on fingerprinting on file before the first day of employment. Evidence: Staff A (start date: 6/27/22) did not have documentation of a fingerprint determination letter on file.

Plan of Correction: Staff A will complete another fingerprint check if the original results are unable to be found.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of four staff records it was determined that not all records contained documentation of a central registry check within the first thirty days of employment. Evidence: Staff A (start date: 6/27/22) did not have documentation of a central registry check on file.

Plan of Correction: Staff A will complete another central registry check if the original results are unable to be found.

Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening before the first day of employment. Evidence: Staff A (start date: 6/27/22) did not have documentation of a TB screening on file.

Plan of Correction: Staff A will obtain a TB screening and submit the results to administration for their file.

Standard #: 8VAC20-780-340-F
Description: Based on observation it was determined that children under 10 years did not always remain in sight and sound supervision of staff. Evidence: The LI (Licensing Inspector) observed the Junior Kindergarten classroom lining up in the hallway to go back to their class from using the restroom. After the children began walking to their classroom the LI entered the restroom for inspection. The LI observed child A (DOB: 3/12/18) washing their hands. Staff had already left the restroom and returned to their classroom. Staff were not aware that child A remained in the restroom until the child was returned to the class by the director. The entire incident from start to finish was less than one minute.

Plan of Correction: Staff will confirm attendance of all children before leaving the restroom.

Standard #: 8VAC20-780-570-E
Description: Based on observation of the infant room it was determined that not all prepared formula was labeled with the child's name and date. Evidence: Three bottles were observed without a child's name or date in the infant room refrigerator.

Plan of Correction: All bottles will be labeled with children's name and date.
