Technical Assistance:
Consultation was given on TB being required within one month prior to hire or the day the employee starts per the updated standards 10/21/2021. Injury forms must be signed by 2 people (parent and staff or 2 staff) and Staff #4 completing VA orientation course by the end of November 2022.

Comments:
An unannounced monitoring visit was conducted from 11:00-12:30. 4 classrooms were in operation with staff. Observed infant care, nutrition services, physical space, transitions to the playground and record reviews. Emergency preparation is current. Lunch to be served today was chicken leg, squash, bread & butter and milk. Children under 1 years of age bring their food from home. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-M
Description: Based on staff statement and observation, the center did not maintain a current list of all children's allergies, food preferences or sensitivities in each room where children are present.
Evidence:
The administrator stated that no list was maintained.

Plan of Correction: Office maintained a list, however we didn't know that new regulation require a new form. It is in compliance as of 10/17/2022.

Standard #: 8VAC20-780-240-A
Description: Based on file review and documentation, 1 staff did not complete the VA Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence:
Both staff #1 was hired 1/2022 and do not have documentation that the 10 hour orientation course has been completed.

Plan of Correction: Staff completed on 11/6/2022.

Standard #: 8VAC20-780-270-A
Description: Based on observation, one area of the center was not maintained in a clean, safe or operable manner.
Evidence:
A door way in the hall was split and could cause a pinching hazard.

Plan of Correction: Door was due for replacement by the Property Management and was replaced on 11/4/2022.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method to prevent access by children.
Evidence:
Bleach wipes were placed on a cubby unit in the twos classroom and toddler classroom and not locked in a cabinet.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and staff statement, the staff to child ratios were not maintained.
Evidence:
10 infants:2 teachers (ratio is 4:1)
9 two year olds:1 teacher (ratio is 1:8)
13 preschool children:1 teacher (ratio is 1:10-this was exceeded from 11:00-11:20)

Plan of Correction: Two teachers were sick that day and couldn't come to work. However, they are back now.

Standard #: 8VAC20-780-510-E
Description: Based on observation, the center does not have written authorization for 2 emergency medications.
Evidence:
Child #1 has 2 emergency medications on site and there is no written authorization completed by the parent or physician for medication on-site longer than 10 days.

Plan of Correction: These two forms were with parents and they have returned the completed forms as of 10/27/2022.
