Comments:
A renewal inspection was conducted on October 12, 2022 from approximately 8:50 AM to 11:35 AM. There were 30 children present, ranging in ages from infant to five years, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. The provider stated that transportation is provided and the vehicle and transportation documents were reviewed. Medication is administered, and medication, authorization forms and allergy plans were reviewed. The annual staff training hours and annual fire and health inspections were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening (TB) at the time of employment, prior to coming in contact with children and been completed within the last 30 calendar days of the date of employment.

Evidence:
1.The record for staff #4 (employed 8/19/22) had a TB screening/test dated 8/23/22. The TB documentation shall be submitted at the time of employment and prior to coming in contact with children.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In classroom #2, one container of Clorox wipes labeled " Caution" and "Keep Out Of Reach of Children" was located on top of the cubbies.
In classroom #3, the diaper changing area had Lysol and disinfecting wipes labeled "Caution" and " Keep Out of Reach of Children" accessible to children.
In classroom #5, hanging from the door was 1 container of glass cleaner and 2 containers of Lysol spray labeled "Caution" and "Keep Out of Reach of Children" and one container of bleach and water.

Description: Based on observation, the program did not have all of the required non-medical emergency supplies.

Evidence:
The battery operated radio did not turn on during the inspection. The director was unable to locate batteries for the radio at the time of inspection.
