Comments:
A monitoring inspection was conducted on 10/19/2022 as a result of a self-reported incident called in by the District Manager on 10/13/2022. A tour of the facility was conducted, in addition to a record review and interviews with staff Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on staff interviews, observation via video footage and record review the center did not ensure the center's activities and services were maintained in compliance with these standards and the center's own policies and procedures that are required by these standards.

Evidence:
On 10/13/202 the center did not follow their own policies and procedures, which resulted in a 2 year-old child being left on the playground in the rain for approximately 10 minutes.
1. The center's supervision policy includes a "Face to Name" form that is to be completed by staff assigned to the classroom to ensure every child is accounted for. The policy states it is to be completed by staff on the following occasions: a). children arriving or leaving the classroom b). transitioning form location to another (playground, field trip, etc) c). combining or blending classrooms d). teacher changes/breaks

2. Video footage showed the 2 year-old class enter the playground with Staff B and C. Neither staff B or C filled out a face-to-name form when crossing the gate and onto the playground area.

3. Video footage showed Staff A coming onto the playground to relieve Staff C. The face-to-name sheet was not completed at that time.

4. At approximately 10:25am, it began to rain. Instead of taking the children through the 2 gates that lead directly to their classroom, Staff B gathered the children and led them through the preschool door, blending the 2 classes. The face-to-name count was not completed before or after blending the classes.
5. Staff A, who was the last to arrive back into the twos class, asked Staff B if the "face-to-name" was filled out. Staff B replied yes.

6. The face-to-name sheet shows Staff B checked all children, including Child #1, without having followed the procedure of looking at the child's face and calling their name before checking them as present.

7. At 10:35am staff D and E saw Child #1 on the playground in the rain, with no supervision.

Plan of Correction: Current two year old staff have all been retrained on proper face to name supervision, as of 10/20/22. No out-of-classroom transitions are occurring without management oversight at this time. All school staff will be retraining on face to name oversight on 11/17/22.

Description: Based on interviews and record review the center did not ensure all children under 10 years were always within sight and sound supervision of staff.
Evidence:
On 10/13/2022 at approximately 10:25am a 2 year-old child was left on the playground in the rain until approximately 10:35am.

Plan of Correction: Current two year old staff are observed and coached in the moment daily on best practices for direct supervision of children. This also includes camera observations, the implementation of walkie/talkie phones for staff to contact management either from the playground or the classroom at anytime. Staff use an assigned school phone to notify school management anytime they transition to the playground for direct oversight of this process for now. District Manager is on site daily to assist with this oversight and additional training.
