Comments:

An unannounced monitoring inspection was conducted on 11/01/2022. There were 44 children, ages 8 weeks-5 years, 9 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in learning activities, free choice activities, and during lunch. The inspector discussed the following with the provider: infant requirements, emergency evacuation drills and record keeping. A total of 5 children?s records, 5 staff records and 2 medications were reviewed. The inspector arrived for the inspection at 10:15 am and departed at 2:10 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	22.1-289.035-B-4
Description:	Based on record review, the center failed to obtain a copy of a criminal history record check and a copy of a child abuse and neglect search from any state in which the individual has resided in the preceding five years. Evidence: The record for Staff 3 contained documentation that Staff 3 has resided outside of the state of Virginia within the preceding five years. The documented hire date for Staff 3 was 08/03/2022. There was not a child abuse and neglect search from all of the states in which the individual had resided in the preceding five years available for review during the inspection.

Description:	Based on record review, the center failed to ensure that each staff record contains all of the elements as required by the standards. Evidence: 1. The record for Staff 2 did not contain the following information: job title, date of employment; and documentation that the individual possess the education, certification and experience required by the job position. 2. The record for Staff 3 did not contain the following information: job title; and documentation that the individual possess the education, certification and experience required by the job position. 3. The record for Staff 4 did not contain the following information: job title; and documentation that the individual possess the education, certification and experience required by the job position. 4. The record for Staff 5 did not contain the following information: job title, date of employment; and documentation that the individual possess the education, certification and experience required by the job position.

Standard #:	8VAC20-780-500-A
Description:	Based on observation, the center failed to ensure that staff shall wash their hands with soap and running water before feeding or helping children with feeding. Evidence: 1. Staff 6 was observed feeding two children from separate baby food containers at the same time; giving one child a bite from a container/spoon and then feeing the next child. Staff 6 did not wash their hands with soap and running water before feeding each child. 2. Staff 6 was observed immediately picking up another child and feeding that child a bottle after feeding other children. Staff 6 did not wash their hands with soap and running water before feeding that child.
Plan of Correction:	Will retrain the staff.
Standard #:	8VAC20-780-500-B
Description:	Based on observation and staff interviews, the center failed to ensure that the diapering area shall have a sink with running warm water. Evidence: 1. The portable sink in the 2 Year Old Classroom did not have warm running water. The tank in the portable sink was empty. The portable sink does not have the ability to heat the water to provide the diapering area with running warm water. Staff 2 stated that they had changed diapers in the diapering area on 11/01/2022. 2. The portable sink in the 12 month-18 month Classroom did not have warm running water. The portable sink does not have the ability to heat the water to provide the diapering area with running warm water. The staff verified that they had changed diapers in the diapering area on 11/01/2022.
Plan of Correction:	Will move the diapering area until new portable sinks arrive. The sinks will be ordered today.
Standard #:	8VAC20-780-560-G
Description:	Based on observation and staff interviews, the center failed to ensure that when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: There were three unlabeled, undated sippy cups on a shelf in the 2 Year Old Classroom. Staff 7 verified that the cups were brought from home.
Plan of Correction:	The front staff will check all bags upon arrival and label/date all food from home.
