Comments:
An unannounced on-site monitoring inspection was conducted on 10/04/2022. There were 61 children, ages 4 months-5 years, and 15 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and painting. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on review of five children?s records the center failed to ensure that all children?s records were complete.
Evidence:
1. Child # 1 has a diagnosed food allergy and did not have the required food allergy action plan.

Plan of Correction: The Director will have the parent to obtain the Action Plan. In future all diagnosed food allergies will have an action plan.

Description: Based on observation, the center failed to ensure that all chemicals were locked as required.

Evidence:
1. Observed in the older 2 year old room a container of sanitizer was sitting on a cubby.
2. Observed in the three year old sitting on a high cabinet a container of cleaning chemicals that was not accessible to the children but unlocked.

Plan of Correction: The staff locked all chemicals during the inspection.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that all playground equipment that required resilient surfacing had the required amount.

Evidence:
1. Directly under the playground equipment there was no measureable resilient surfacing. The playground equipment was moved off the resilient surfacing and was sitting on a wooden deck.

Plan of Correction: The piece of equipment will be moved back to the resilient surfacing and will not be used unless it has the surfacing under it.
