A self-reported incident inspection was initiated on 10/24/22 and concluded on 10/24/22. Information was gathered and reviewed regarding the incident.
This inspection determined a violation of standards did occur, and is noted on the violation notice.

Standard #: 8VAC20-780-410-1 Description: Based on review of video and discussions with staff the center failed to ensure that staff do not use forbidden actions of roughly handling or shaking a child as behavior guidance.
Evidence:
On 10/21/22 the director of the center self-reported, to licensing, an incident of a staff roughly handling and shaking a child. Video was provided for review.
The video showed staff #1 roughly grabbing the child from the table he was climbing on and placing him on his bottom on the floor forcefully, grabbing his shirt on both shoulders while shaking the child twice , while being in his face, nose to nose, reprimanding him.
The staff let go of the child and he moved onto a nearby cot when staff #1 grabbed the child up again forcefully carrying him under his arms, in front of her, to the corner of the room and forcefully placing him on his bottom against the wall and shaking him again causing his back to hit the wall, while being in his face at the same time.
The staff member left the child, and the child got up and went to another area of the room, where staff #1 again grabbed the child up forcefully and carried the child to the same time-out spot, forcefully placing his bottom on the floor and getting in the childs face again.
The child got up and moved to a near by cot and staff #1 again came over to the child knelt down and got into the childs face.
During the 3 minute incident staff #1 used forbidden action for behavior guidance by forcefully grabbing the child 3 different times; shaking the child 4 different times; jerking the child around 3 different times; forcefully placing the child on his bottom3 different times; and being nose to nose in the childs face 4 different times.
There was no audio in the video to hear what the staff was saying to the child during the times the staff was in the childs face.Plan of Correction: Once administration was notified of this incident, on 10/21/22, the video was immediately reviewed. Administration immediately terminated and removed the staff from the classroom and building. Administration contacted licensing to self report this incident. All staff have been reminded of appropriate behavior guidance, and the policies and procedures of the center. The center does not tolerate breach of these practices and policies.
