Technical Assistance:
Licensing inspector discussed an intensive plan of correction with the director.

Comments:
An unannounced monitoring inspection was conducted today from approximately 2:40pm-3:35pm in response to self-reported incident. Observations were made, documents were reviewed, and staff were interviewed. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations:
Standard #: 8VAC20-780-40-M
Description: Based on record review, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan.,

Evidence:
The record of Child A stated that Child A was allergic to rice, oats and bananas. These items were missing from the most recent allergy list.

Plan of Correction: - Items in question, added to list immediately

-Will set calendar reminder to update lis monthly, as well as with each time a new allergy is noted with a child.

Standard #: 8VAC20-780-340-A
Description: Based on interview and record review, when staff were supervising children, they did not always ensure their care and protection.

Evidence:
1) Staff A fed Child A food that listed on the allergy plan of Child A.
2) After a conversation with the parents, Staff B stated that Child A vomited twice once they were home.

Plan of Correction: Specifically re-trained Staff A on the importance of the Allergy List and knowing who in the classroom brings their own food because of their allergy or sensitivity.

Standard #: 8VAC20-780-340-D
Description: Based on interview, in each grouping of children at least one staff member who meets the qualifications of a program leader was not regularly present.

Evidence:
Staff B stated that the infant room does not have a lead teacher regularly present.

Plan of Correction: We have re-assigned a teacher to be the Lead for the class and have a second staff member in the CDA program that will be completed by Mid-March 2023.

Standard #: 8VAC20-780-560-P
Description: Based on interview and record review, a child with a diagnosed food allergy was served food identified in the written care plan.

Evidence:
1) The record of Child A states Child A is allergic to oats and bananas.
2) Staff A stated that she served Child A three tablespoons full of oatmeal and three bites of banana.

Plan of Correction: Retrained all staff on the allergy and food preference list in the classrooms.
