The purpose of today?s visit was to monitor compliance during a routine unannounced compliance visit.
Upon my arrival, I was greeted by the Director. A walk-through of the center was conducted. The children were observed during individual activities, transitioning to outdoor play, circle time and resting. The following items were observed in compliance during today?s visit: proper storage of
hazardous materials, use of approved space, physical plant, administration, and programs. A total of 5 child records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Based on staff records review, the center did not obtain documentation of a negative
tuberculosis screening at the time of employment and prior to coming into contact
with children.
Evidence:
1. Staff #2 (date of employment 9/12/2022) did not have documentation of TB screening in

Standard #: 8VAC20-780-520-A Description: Based on review of over-the-counter products in the classroom, the center did not comply that
over-the-counter skin products shall not be kept or used beyond the expiration date of the
product.
Evidence:
1. In the two year old classroom, Child #1 had two over-the-counter skin products that were
kept beyond the expiration date of the product; Aquaphor expired on 11/2021 and Aveeno
