Technical Assistance:
Technical assistance was offered related to:

1. Medications and over the counter ointments; forms for allergies and medications
2. Background checks

Comments:
An unannounced monitoring inspection was conducted between 10:45 am to 12:45 pm. There were 81 children with 11 staff present. The children were engaged in structured learning activities, playing outdoors and enjoying a birthday celebration. Medications and emergency drill records were reviewed. 3 staff records and 3 children's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on records review, written care plans for children with a diagnosed food allergy were not included in the child's record.
Evidence: Child # 1, Child # 2, Child # 3 and Child # 4's record indicated that they have diagnosed food allergies. A written allergy action plan was not included in their record.

Plan of Correction: Given action plan with medication form to parents.

Standard #: 8VAC20-780-510-L
Description: Based on observation, medications were not kept locked and were accessible to the children.
Evidence:
1. The safety bag for the Explorer's classroom was seen hanging on the low fence (approximately 3-4 feet high) of the playground while the class was using the playground.
2. Child # 2's emergency medication was stored in a ziploc bag and kept in the bag which was not locked.
3. The medication was within reach of the children.

Plan of Correction: A command hook has been purchased and placed outside out of reach of children.

Standard #: 8VAC20-780-520-A
Description: Based on observation, an over the counter product was used beyond the expiration date.
Evidence:
1. Child 5's first day of enrollment was 11/8/21.
2. Child # 5's diaper rash cream had an expiration date of 10/2020.

Plan of Correction: Product was thrown out.
