Comments:
An unannounced renewal inspection was conducted on 11/2/22 from 9:30am to 1:00pm with the Director. There were 92 children in care, ranging in age from six-months to five-years-old, supervised by 15 staff. The children were observed participating in circle time and talking about how they were feeling; having diapers changed and listening to a story; having free play in centers; talking about colors and playing a color matching game; and playing on the playground. Six child records and six staff records were reviewed. The Center has 18 staff with current certification in CPR and First Aid, and eight staff trained in Daily Health Observation. Four medications and corresponding authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. The most recent Fire Inspection on file was dated 9/29/22 and the most recent Health Inspection on file was dated 1/15/21. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 11/11/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on review of six child records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The immunizations on record for Child B, with a start date of 6/24/22, were dated 6/29/22. The immunizations on record for Child C, with a start date of 9/16/22, were dated 9/28/22. The immunizations on record for Child D, with a start date of 4/27/22, were dated 5/17/22. The immunizations on record for Child F, with a start date of 10/18/22, were dated 10/26/22.

Plan of Correction: Required immunizations will be obtained prior to a child's start.

Standard #: 8VAC20-780-240-E
Description: Based on review of six staff records, the center did not ensure that staff completed orientation training in first aid and CPR within 30 days of the first day of employment, when required. Evidence: The records for Staff #1, with a start date of 9/12/22, and Staff #2, with a start date of 3/14/22, did not contain documentation of first aid and CPR orientation.

Plan of Correction: Moving forward all staff will receive CPR and First Aid orientation within 30 days if they do not already hold a current certification.

Standard #: 8VAC20-780-260-B
Description: Based on review of documentation and interview, the center did not obtain annual approval from the local health department. Evidence: The most recent health inspection on record was dated 1/15/21.

Plan of Correction: The Health Inspection has been scheduled.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that equipment of the center was maintained in a safe condition. Evidence: There were five tricycles observed on the playground that had large holes with jagged edges in the seats.

Plan of Correction: The tricycles were removed from the playground during the inspection and will not be returned to the playground until they are repaired.
