Comments:
An unannounced monitoring inspection was conducted on 11/09/2022 from 9:45 am - 12:00 pm. At the time of entrance there were 30 children in care, ages 16 months - 4 years, with 4 teaching staff. Children were observed during morning program time and during outdoor play.
Records were reviewed for two new staff and four children.

Information gathered during the inspection determined non-compliances with applicable standards and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on record review, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. Attendance had not been taken for the toddler II class when the attendance record was reviewed at 10:00 am.
a. six toddlers were in care at this time and the teacher for this class confirmed she had not taken attendance yet.

Plan of Correction: Attendance was taken during the inspection. All staff will be reminded that attendance is to be noted as the child arrives. The program director indicated that the attendance sheet will be updated to make it easier for staff to note the arrival of children when they come into the classroom.

Standard #: 8VAC20-780-510-P
Description: Based on medication review, the center failed to ensure when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Evidence:
1. The written authorization for one over-the-counter medication expired 09/26/2022.
a. This medication was in the facility locked medication box.
b. The program director confirmed that an updated authorization had not been obtained because the medication was not being administered.

Plan of Correction: The program director stated this medication would be returned to the parent today.
