Comments:
A renewal inspection was conducted on 11/04/2022 with the center director and staff. There were 29 children present, ranging in ages from 5 months to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The children were preparing for lunch, eating, and preparing for rest.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of four staff files, the facility did not ensure that the required fingerprint check was completed within the last 90 days prior to the date of employment. Evidence: The fingerprint check for Staff A (date of re-hire 10/15/2022) was dated 9/28/2018. Staff A had been on leave from the facility for more than one year, requiring a new fingerprint check.

Plan of Correction: Staff will obtain a new fingerprint check.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of four staff records, the facility failed to have documentation of a completed search of the central registry (CPS check) on file for each staff within the first 30 days of employment. Evidence: Staff D (date of employment 5/10/2022) did not have a completed search of the central registry on file. There was no documentation of having followed up on the CPS check when it was not returned within 30 days.

Plan of Correction: The center director will follow up with CPS regarding the background check.

Standard #: 8VAC20-780-140-A
Description: Based on review of five children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 2 (date of enrollment 9/23/2022) did not have documentation of a physical in the file.

Plan of Correction: Center director will request the physical from the parents.

Standard #: 8VAC20-780-280-G
Description: Based on observation and interview, the facility failed to clearly label the contents of of substitute containers that contain hazardous substances. Evidence: a spray bottle in the infant room cabinet was not labeled; the staff identified the contents as bleach water.

Plan of Correction: All substitute containers will be labeled.

Description: Based on observation, the facility failed to maintain the required ratio of one staff for every 8 children aged 2 years. Evidence: Staff C was alone with 9 children ages 2 years and 3 years.

Plan of Correction: Children were out of school, which made rooms out of ratio.

Standard #: 8VAC20-780-350-C
Description: Based on observation, the facility failed to maintain the required ratio of the youngest child in the group when the children are in mixed age groups. The required ratio for is one staff for every 4 children ages birth to 16 months. Evidence: Staff E and Staff F were alone with 12 children ages 5 months to 2 years from when the inspector arrived at 11:40 a.m. to 1:00 p.m. when a third staff arrived.

Plan of Correction: The third staff was running late due to car trouble.

Standard #: 8VAC20-780-560-F-4
Description: Based on observation of lunch in the infant and toddler room, the facility failed to offer children under the age of three years foods that are not considered a choking hazard. Evidence: Hot dogs cut into rounds were served in the infant and toddler classroom to children 12 months to 2 years.

Plan of Correction: Hot dogs will be cut lengthwise or in quarters for children 3 and under.
