Technical Assistance:
Please ensure that your topical ointments are located in an area that is not accessible to children and monitored. This was found at the last licensing inspection. A shelf located above the diapering area may be acceptable.

Clarified the need that 1 person with current Daily Health Observation training must be onsite at all times.

Discussed the need to contact the local building official for inspection of the newly installed wall in your building.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 17 children with 3 staff onsite. Two classrooms occupied today. Observation of both classrooms, programming, playground and unoccupied classroom was made. The children were observed having morning snack in the two year old classroom and in the mixed age classroom, children were engaged in both individual activities and small group activities. Positive interactions and guidance was observed between staff with children. One medication onsite. All staff records were reviewed and 4 children's records and 1 medication authorization was reviewed. This inspection occurred between 9am and 12:10pm. Thank you for your cooperation today. Standard #: 8VAC20-780-130-A
Description: Based on review of children's records, documentation of immunizations were not obtained before one child began care.
Evidence:
Child #1 began care on 4/13/21. Documentation of immunizations were dated 5/4/21.

Plan of Correction: No child will begin care until immunization documentation is obtained.

Standard #: 8VAC20-780-160-C
Description: Based on review of staff records, a subsequent TB screening was not obtained for one staff.
Evidence:
Staff #3's last documented TB screening was dated September 2019.

Plan of Correction: Ongoing, children will not begin care unless immunizations are obtained from parents.

Standard #: 8VAC20-780-90--A
Description: Based on child record review, a written agreement was not found in one child record.
Evidence:
The parent handbook acknowledgement that includes the center's sick policy and requirement to report communicable diseases was not found in Child #1's record.

Plan of Correction: The parents will sign the parent handbook disclosure.

Description: Based on review of staff records and staff interviews, a staff member with daily health observation training was not on duty.
Evidence:
A total of 4 staff onsite, no staff on duty had current Daily Health Observation training.

Plan of Correction: We will enroll into this training as soon as possible.

Description: Based on observation of the center, written documentation from the local building official was not obtained prior to use of one area of the building.
Evidence:
A newly installed wall was build enclosing the 2 year old classroom. The new construction was not approved by the local building official.

Plan of Correction: The contractor is following up with the building official to obtain written approval for this construction. Will forward this to the Licensing Inspector for review.

Standard #: 8VAC20-780-340-F
Description: Based on observation of the 2 year old class, children were not always within actual sight supervision of staff.
Evidence:
Staff #1 left the 2 year old classroom leaving 4 children out of sight supervision for approximately 10 seconds.

Plan of Correction: This was corrected immediately. Staff will be reminded that all children must remain in sight at all times.

Standard #: 8VAC20-780-420-E-3
Description: Based on child record review, an annual review of 2 child records was not found.
Evidence:
Child #1 and #2's record was not reviewed annually.

Plan of Correction: Parents will review the records and document the review.

Standard #: 8VAC20-780-520-C
Description: Based on observation of the center, topicals were accessible to children in care.
Evidence:
Topical ointments were found accessible to children in the bathroom located in the 2 year old classroom.

Plan of Correction: The topical ointments were relocated and made inaccessible. Owner will continue to monitor the storage of these ointments to ensure this is not repeated again.

Standard #: 8VAC20-780-560-L
Description: Based on observation of the 2 year old class, staff did not sit with children during one meal time.
Evidence:
Staff #1 did not sit with the children during morning snack time.

Plan of Correction: This staff will be reminded that they must remain with children while they eat.
