Comments:
A renewal inspection was conducted on November 15, 2022 from 9:40 am until 11:30 am. There were sixteen children present, ranging in ages from two to five years old, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of three staff files, program failed to ensure that a copy of the results of a criminal history record check from any state in which the individual has resided in the preceding five years was obtained prior to the first day of employment.

Evidence:
1. The first day of employment for Staff #3 was 11/1/2022. There were no criminal history results in the file for a previous state of residence.
2. The Director confirmed that she had received criminal history results for Virginia prior to the start of employment, but had not requested results from the previous state of residence.

Plan of Correction: An out of state central registry check was done for the staff member but an out of state criminal history check was not done. In the future, all staff members from a previous state will have both a central registry clearance and a criminal background check done on or before his/her start date. An out of state background check has been completed for this staff member. Elizabeth Claman is responsible for this action.

Standard #: 8VAC20-780-560-G
Description: Based on observation, program did not ensure that food and drink brought from home is dated and labeled in a way that clearly identifies the owner.

Evidence:
1. During snack time on 11/15/2022, an unlabeled, reusable thermos was identified. Staff questioned one another to see if anyone knew who the owner was. When they were not certain of the owner, it was placed on top of the cubbies and was not given to any child.
2. The Director confirmed that it was not labeled and that they tell parents it is required, but that it was a new thermos.

Plan of Correction: Currently all the children's lunch boxes are given a dated sticker on a daily basis. Water bottles, snacks, and lunch boxes are labeled by parents. In the future, all water bottles will be checked to ensure they have a label before snack/lunch time. Parents have been sent an email today to please label lunch boxes, snacks, and water bottles. Parents will be reminded on a quarterly basis. Elizabeth Claman is responsible for this action.
