A monitoring inspection was conducted on 11/16/2022. There were 18 children present and 2 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-260-A Description: Based on a record review the center did not have documentation of having an annual fire inspection.
Evidence:
The center's last fire inspection was dated 09/22/2022.Plan of Correction: The city has the fire inspection and has not provided the copy yet. The director will talk to administration to follow-up on this.
Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas of the center were maintained in a safe condition.
Evidence:
The closed-off fireplace ledge had peeling paint on the area that children sit on.Plan of Correction: The director will let administration know and have them notify parks and recreation that this is not in compliance.
Standard #: 8VAC20-780-280-B Description: Based on observation the center did not ensure all hazardous substances were kept in al locked place using a safe locking method that prevents access by children.
Evidence:
The was a bag of charcoal briquets on the floor in the children's game room.Plan of Correction: The bag of charcoal was immediately removed. It belonged to another department that left it in the common area.
