Technical Assistance:
Make sure all staff are aware of the location of the allergy/ sensitivity/dietary restriction written list.

Comments:
An unannounced inspection was conducted with the two teachers present. At the end of the inspection there were 15 preschoolers. The inspection took place from 8:40 am to 10 am. The Licensing Inspector was able to observe drop off time and free play.
Six children and three staff files were reviewed. For breakfast, the teachers reported that the children were service cereal and milk. There was no medication on site.

Violations:
Standard #: 8VAC20-780-90--A
Description: Based on a review of children's files, it was determined that the parental agreement did not cover all of the required areas.
Evidence:
Six children's files were reviewed, there were no signed agreements for child A, B, C, D and E where the parents agreed to inform the center within 24 hours of a household member having a contagious illness, and they will pick up if notified that their child is ill. Child A enrolled on 11/2/20, child B enrolled on 1/18/22, child C enrolled on 8/22/22, child D enrolled on 10/15/21 and child E enrolled on 11/30/20.

Plan of Correction: We will review all children's files to make sure parents have signed off on all components of the agreement.

Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that hazardous substances were not locked.
Evidence:
1. The Licensing Inspector (LI) observed an unlocked bottle of bleach water solution in the bathroom on a shelf above the toilet ;
2. The LI observed a bottle of hand sanitizer sitting on a shelf in the office. the hand sanitizer was accessible to children;
3. The LI observed glass cleaner and flea & tick aerosol in the office unlocked;
4. The LI observed on the outside window seal a disinfectant spray. This spray was unlocked and accessible to children on the playground.

Plan of Correction: We locked these hazardous substances and will retrain staff on the importance in keeping these items locked.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, it was determined that 6 inches of resilient surfacing was not maintained nor was their six feet of fall zone maintained for the outside pieces of equipment;
Evidence:
The Licensing Inspector observed that the mulch surrounding the outside play equipment had degraded and has turned to dirt, which is not an acceptable resilient surface.
The Licensing Inspector observed the 5 seat helicopter see saw within 1 foot of the stationary caterpillar. The red slide was within 3 feet of the caterpillar. The see saw was within 3 feet of the helicopter. These moving an climbing pieces of play equipment requires at least 6 feet of fall zone free of any other obstacle.

Plan of Correction: We will add six inches of mulch and rearrange the playground to ensure equipment is not in the fall zone of equipment. We will place the equipment off limits with signage and tape until brought into full compliance.
