Technical Assistance:
Consultation provided on documentation of tummy time, annual lockdown drills, listing on the injury form, the specific time of day parents are told of injuries/accidents, blowing the leaves off of the playground as they can become slippery when wet, and components of the written allergy/dietary restriction/sensitivity list.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with a total of 54 children with 11 staff providing supervision. The ages of the children present ranged from 5 months to 5 years. The inspection took place from 10:25 am to 12: 40 pm

The children were engaged in a variety of activities to include music and movement, morning sensory activities with shaving cream, table top games, free play and outside play.

Once child had two medications on site as well as a written allergy care plan.
A sample of 7 injury/accident reports were rev iewed.
Four staff and six children's files were reviewed.
The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of six children's files and interview with the director, it was determined that child C did not have immunization record on file. Immunizations are due upon enrollment and child C has been enrolled since 9/6/22.

Plan of Correction: The family let us know that the child has a doctor?s appointment on 11.18.22 and will be providing a copy of the most recent and updated immunization record. For the rest of the school, we will be thoroughly auditing each child that is currently on fire to make sure that each child has most updated and current immunization record on file. As of this morning, mom brought in the current immunization form.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, it was determined that one has an allergy , but was not listed on the written allergy/sensitivity/dietary restriction list.
Evidence:
Child A was diagnosed with an allergy to Amoxicillin but was not listed on the allergy list. Child A enrolled on 7/25/22.

Plan of Correction: We must also conduct an inquiry on student files to make sure that
this food preference is stated on a child?s enrollment file as well. Going forward we will inspect an enrollment form thoroughly before a child?s start date with parent preferences to make sure that it states the food preference and confirm with said parent, add the child to the allergy list with stated food preferences, then provide a copy of the allergy list to the whole school so that they are aware.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of recordkeeping and interview, it was determined that child B has a diagnosed food allergy to grapefruit, pomegranate and tap water, but there was no written allergy care plan on file.

Plan of Correction: The plan of action for this will be to provide a plan of action sheet that our building uses to the child?s parent to be filled out immediately and then given to the doctor if need be to complete the form. In the future, each child who has a diagnosed allergy will be given an allergy care plan sheet upon enrollment or upon a new diagnosis conducted by a medical professional and was confirmed with a pediatrician. The child will then be placed on our buildings allergy list and each classroom will be provided an updated copy with the recently added child onto the allergy list.
