Comments:
An unannounced monitoring visit and facility tour was conducted on 11/18/2022. At the time of inspectors' arrival there were 24 infant to preschool aged children in care with 5 staff members and 1 administrators. Children were observed interacting with staff, nap, and free play. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Director reported that there medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Description: Based on record review and interview it was determined the center did not ensure all staff submitted to fingerprinting prior to hire.
Evidence:
1. There was no documentation of criminal history search in the records for staff #1(hired 8/30/22), staff #2(hired 11/8/22), staff #4(hired 10/11/22), staff #4(hired 9/6/22), and staff #5(May 2022).
2. Staff confirmed these documents were not available during this inspection.

Description: Based on record review and interview it was determined the center did not request the required out of state background checks within the required timeframes.
Evidence:
1. The records for staff #1(hired 8/30/22) and staff #4(hired 9/6/22) did not contain documentation of the required out of state background checks, as both staff indicated they have lived outside of VA within the past 5 years.

Description: Based on observation an interview it was determined that the center did not ensure that there was 1 staff member to every 4 children aged birth-16 months.
Evidence:
1. There were 7 children in the infant class with 1 staff person.
2. Staff #4 reported that staff #1 was on their break.
