Comments:
A complaint inspection was completed on 11/18/2022. The inspector reviewed compliance in the areas of programs, background checks. A total of 11 child records and four staff records were reviewed. The inspection started at 12:15 am and concluded at 2:24 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Complaint related: No
Description: Based on record reviews, the center failed to ensure that staff who have worked outside of the state of Virginia in the past five years had a request sent for a child abuse and neglect search from that state by the end of the 30th day of employment.

Staff #6 (hire date 8/8/22) does not have an out of state child abuse and neglect request from the state listed on the sworn statement indicating the staff person had lived in another state within the last five years.

Plan of Correction: Follow up with out of state application to find results asap. [sic]

Description: Based on record reviews, the center failed to ensure that central registry checks are received by the end of the 30th day of employment.
Evidence:
1. Staff #3 (hire date 6/6/22, central registry received 11/11/22), staff #5 (hire date 7/26/22, central registry received 11/15/22), and staff #6 (hire date 8/8/22) do not have a central registry check on file within 30 days of employment.

Plan of Correction: Follow up with Virginia CPS background check office to find results asap. [sic]

Description: Based on interviews, the center failed to ensure that the following actions are forbidden: Punishment by applying unpleasant or harmful substances.
Evidence:
1. During an investigation on 9/13/22, the inspector found an open apple cider vinegar bottle in the staff?s training/break area. Staff #1 and #2 stated that it was sprayed on the child?s arms to stop biting. They added that it had not been used in a while. During further interviews with staff, it was determined that staff spray apple cider vinegar in children?s mouths if they bite.
2. The LI observed written permission provided by three parents allowing the center to spray apple cider vinegar in their children?s mouths for biting.
3. Staff #1 admitted to spraying the apple cider vinegar in two children?s mouths.

Plan of Correction: Apple Cider vinegar use was not being used at time of inspection and bottle was thrown away during investigation. It will not be used again. [sic]
