Comments:
An unannounced non-mandated monitoring inspection, required by the Conditional License, was conducted on-site on 11/15/2022 and concluded virtually on 11/21/2022. There were 11 children, ages 4 years-10 years, and 2 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, programs, and special care provisions and emergencies. A total of 5 children?s records and 3 staff record were reviewed. The children were observed arriving to the program, having an afternoon snack and during homework time. The inspector arrived to the center at 2:30 pm and departed at 4:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on observations and record review, the center failed to ensure that operational responsibilities of the licensee shall include ensuring that center?s activities, services, and facilities are maintained in compliance with the terms of the current license issued by the department.

Evidence: The current license issued by the department allows for the care of children ages 4 years 11 months to 12 years. The review of the documented birthdate for Child 1 determined that Child 1 was 4 years 3 months old. Child 1 was in attendance during the 11/15/2022 inspection.

Plan of Correction: Requesting a license modification to care for children 4 years to 12 years.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.

Evidence: The record for Child 1 did not contain the following information: name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the second designated person did not list an address); and the first date of attendance.

Plan of Correction: Will get the missing information from the parent.

Standard #: 8VAC20-780-70
Description: Based on record review, the center failed to ensure that staff records contain all of the elements as required by the standards.

Evidence: The record for Staff 2 did not contain the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. Staff 2 was on-site during the 11/15/2022 inspection.

Plan of Correction: Will update the spreadsheet and get that to the site.

Standard #: 8VAC20-780-240-A
Description: Based on record review, the center failed to ensure that staff shall complete the Virginia Department of Education sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence: The record for Staff 3 contained a documented hire date of 07/27/2022; Staff 3 had not completed the Virginia Department of Education sponsored orientation course as of 11/21/2022.

Plan of Correction: Will have the staff member complete the course.
