Comments:
A monitoring inspection was initiated on 11/21/2022 and concluded on 11/21/2022. There were 78 children directly supervised by 17 staff members. Children were observed engaged in group activities, stories, music and movement, manipulatives, playdough, writing letter H and block. The physical plant, 4 child records, 4 staff records, medications, and evacuation drills were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on a review of children record, medication authorization and medication, a written care plan for each child with a diagnosed food allergy has not been obtained from the physician.
Evidence: Child #2 has an epi pen for food allergies, the parent has not provided a written care plan from the physician to include instructions regarding the food to which the child is allergic and the steps to take in an event of an allergic reaction.

Plan of Correction: The parent will obtain a allergy action plan from the physician for the child with food allergies.

Standard #: 8VAC20-780-240-B
Description: based on a review of staff records, there is no written documentation that a staff member has completed orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibility.
Evidence: Staff #1, date of hire 8/19/2019 does not have documentation of orientation training.

Plan of Correction: Administration will complete the orientation with the staff.

Standard #: 8VAC20-780-570-E
Description: Based on observation, Prepared infant formula is not dated.
Evidence: There were 2 infants in the older infant room that had prepared formula bottles that were not dated.

Plan of Correction: Staff will ensure that bottles are dated daily.
