Comments:
The Licensing Inspector conducted an unannounced complaint inspection with another agency in response to a complaint that was received by the licensing office on 11/17/2022. Interviews were conducted with staff relating to the allegations of written accident reports. The classroom was observed, equipment inspected, staff were interviewed and accident reports were reviewed. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Description: Based on staff interviews and a review of incident reports, the center did not maintain a written record of a child's injury the day of occurrence.
Evidence: Staff #1 was interviewed and explained there were 2 separate incidents where she observed the infant fall onto the edge of the wooden toy box. The first incident the infant hit their mouth, their 2 teeth went into the infants gum. The second incident the child fell into the wooden toy box, injuring their upper lip. The staff stated she wrote an injury report for the second incident which is dated 11/7/2022. That injury report for 11/7/2022 was completed and filed in the child's record.
The complainant states the daycare called her on 11/3/2022 saying the infant fell on the wooden toy box hitting his mouth and it was bleeding. The complainant did not receive a written report. Staff #1 and management stated that the parent was called but an accident report was not completed for this incident.

Plan of Correction: Management will remind all staff that accident reports must be completed the day the accident occurs.
