Comments:
A monitoring inspection was conducted on 11/17/2022 between the hours of approximately 8:40 am and 11:00 am. There were 29 children present with 12 staff supervising. Children and staff were observed while supervising children eating breakfast, engaged in conversation and free play with children and playing outside.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review, one staff person did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1) On the date of inspection, the record for Staff #1 (start date 04/26/2022) did not contain documentation of having completed the Virginia Department of Education-sponsored orientation course.

Plan of Correction: Employee will complete training. Employee hired 04/26/2022. Oversight due to employee separating from employment 06/28/2022. She then returned for re-hire 09/06/2022.

Standard #: 8VAC20-780-510-I
Description: Based on medication review, one prescription medication kept in the center was not maintained in the original, labeled container dispensed from a pharmacy.
Evidence:
1) On the date of inspection, a prescription medication (epinephrine injection) for Child #4 was observed without the original container. The label from the pharmacy with the child's name, the name of the medication, the dosage amount, and the time or times to be given was not available for review.

Plan of Correction: Contact parents to request original box with prescription label.

Standard #: 8VAC20-780-510-L
Description: Based on medication review, not all medication was kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) On the date of inspection, prescription medication for Child #5 (epinephrine injector) was observed kept in the classroom inside the emergency backpack, however, the backpack did not have a lock.

Plan of Correction: Lock was given to Child #5's teacher for use on their backpack where medication is kept out of reach of children. Medication pouches will be explored for potential purchase for all rooms moving forward.
