Comments:
A monitoring inspection was initiated on October 26, 2022 at approximately 10:20 AM to 12:30 PM. There were ten children present, ranging in ages from two to four years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records and two staff records were reviewed. A member of management stated there were no medications on site.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A
Description: Based on a review of children's records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
The record for child #3 (start date 8/17/22), did not have a physical examination.

Description: Based on a review of children's records, the center did not ensure that each record contains all required information.

The records for child #5 and child #8, did not contain documentation of viewing proof of the child's identity and age.

Description: Based on staff interview the licensee did not ensure during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.
