Comments:
A mandated monitoring inspection was conducted on November 21, 2022 from 9:45 A.M.-1:00 P.M. There were 40 children present, ranging in ages from 23 months to twelve years of age, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review of staff files, the center failed to ensure that an updated tuberculosis screening/test was completed every two years.

Evidence:
1. Staff #7's tuberculosis screening/test on file was dated 07/10/2020. An updated tuberculosis screening/test was required by 07/10/2022.
2. Administration verified that an updated tuberculosis screening/test had not been obtained.

Plan of Correction: Administration contacted the local health department and got an appointment scheduled for the employee to obtain a tuberculosis screening/test.

Standard #: 8VAC20-780-500-B
Description: Based on observation, a non-absorbent surface for diapering was not used.

Evidence:
1. The diaper changing mat, located on the changing table in the back classroom, had small tears and holes on both sides of the mat. The holes and tears make the diaper changing mat non-absorbent.
2. Administration verified that the mat had small tears and holes on both sides of the mat.

Plan of Correction: Administration ordered a new changing mat. Administration will review with staff the requirement for a non-absorbent surface during diapering, and will be instructed to notify administration when tears or wholes are observed to purchase new mats.
