A complaint investigation was initiated on 10/21/2022 at 11:00am and concluded on 10/27/2022 in response to a complaint received by the licensing office regarding sanitizing tables and equipment, illness notifications and staff to child ratios. Interviews were conducted with staff, policies and procedures were reviewed, and documentation was viewed. Based on the preponderance of evidence collected throughout the investigation, the complaint regarding staff to child ratios was determined to be valid. The exit interview was conducted on 11/22/22 with the director. Standard #: 8VAC20-780-350-B-4 Complaint related: Yes Description: Based on review of written attendance records, count sheets, and staff interviews the center did not maintain the required supervision ratio of staff to children whenever children are in care.
Evidence:
On October 20, 2022 in the Pre-K A classroom, the center was staffed with 1 teacher beginning at 3:30 pm until 5:30 pm. The staff to child ratio for 3 and 4 year olds is 1:10.
1)At approximately 3:30-4:18 there were 16 children present ages 3 years old-4 years old with only 1 staff member present.
2)At approximately 4:18-4:40 there were 15 children present ages 3 years old-4 years old with only 1 staff member present.
3)At approximately 4:40-5:15 there were 14 children present ages 3 years old-4 years old with only 1 staff member present.
4)At approximately 5:15-5:18 there were 13 children present ages 3 years old-4 years old with only 1 staff member present.
5)At approximately 5:18-5:25 there were 12 children present ages 3 years old-4 years old with only 1 staff member present.
6)At approximately 5:25-5:26 there were 11 children present ages 3 years old-4 years old with only 1 staff member present.
